Yes, before I can submit you will need to create an E Form account. You can follow this link:  eForms (atf.gov) 

A 2x2 photo, electronic fingerprints (I do have cards and ink available to purchase) A copy of your Trust paperwork if applicable, a 5320.4 PDF form.
